- 1). Visit the Social Security Administration office in your area to obtain the necessary forms for requesting a copy of your social security card. You may also visit the Social Security Administration's website to download and print the necessary request forms. However, you will need white 8 1/2--by-11 inch paper in order to print the forms. This is the only acceptable method.
- 2). Have proof of your identity available. The Social Security Administration will need to see the original copy (photocopies are not permitted) of your proof of identity. This may include your driver's license, passport, marriage certificate, divorce decree or work permit card, depending on your specific situation.
- 3). Fill out the required forms accurately and completely. Print only in blue or black ink.
- 4). Turn the forms into the Social Security Administration office near you. Bring the required original copies of your proof of identity documents. You may also choose to mail the forms and your proof of identity documents to the Social Security Administration. However, first-time applicants age twelve or older are required to apply in person at the SSA office.
- 5). Wait for your new Social Security card to be mailed to your mailing address. If you need proof of your Social Security card immediately, the SSA office can provide you with a temporary form to use until your Social Security card arrives.
