- 1). Obtain Form 1040X from the Internal Revenue Service website or contact the IRS to have the form sent to you.
- 2). Fill out the top part of the form completely. Provide your full legal name, address and phone number. Do the same for your spouse if you are filing jointly.
- 3). Mark your filing status in section A, regardless of whether you are amending your status. Mark the year for which you are submitting an amended return in section B. Write an explanation of your changes in Section C, such as "amending filing status" or "correcting adjusted gross income."
- 4). Complete the amended return. Provide the proper amounts for wages earned, deductions, credits and tax or refund owed.
- 5). Attach additional forms or schedules that must be redone due to the changes on Form 1040X, such as Schedule C for self-employment income.
- 6). Attach documentation to the form, such as W-2 forms or pay stubs. You must have documentation that supports the changes you made.
- 7). Sign and date form 1040X. Place it in an envelope with the attachments and any payment you owe. Mail it to the IRS.
- 8). Repeat the process with a new Form 1040X for any additional tax year that requires amending.
