On top of the 4 hour schedule, also take your puppy outside after every meal, after waking up and after playing.

When you take your puppy outside to go potty, do not go back inside unless he actually went potty.
Even if that means standing in the rain for 30 minutes.
Keep him on a leash and don't give him the opportunity to think of it as playtime.
When potty training your puppy, it is important to use positive reinforcement.
Did your puppy eliminate outside? Pretend you won the lottery and transfer that happiness and excitement to your pup.
Praise him, give him toys or treats, play with him, pet him.
Basically, make your puppy think that eliminating outside is the best thing in the world.
Keep your eye on your puppy at all times.
If you see him going around in circles and sniffing  the floor, take him outside quickly or clean up later.
It is almost impossible to watch your pup all the time.
Confine him to a small area like the bathroom or kitchen when you cannot watch him.
Crate training might also be a solution to your puppy potty training problems.
Just make sure to learn how to crate train your puppy the right way.
