- 1). Access the United States Postal Service Official Change of Address Form (see Resources). Select whether you wish to forward your mail permanently or temporarily.
- 2). Enter the date you wish to begin forwarding your mail in two-digit month, two-digit day and four-digit year format (mm/dd/yyyy). Use the same format to enter the date you wish to cease forwarding your mail if the forward is temporary.
- 3). Select whether you are forwarding the mail for an individual, a family or a business. Enter your first and last name as well as any suffix if applicable. Enter the name of the person listed as the head of the household if forwarding mail for an entire family. Enter the name of the business if forwarding mail for a business.
- 4). Enter the complete old address you wish to forward mail from and the new address you wish to forward mail to in the corresponding fields.
- 5). Select the box "This residence was built in the last 6 months" if this statement applies to the address to which you are forwarding your mail.
- 6). Enter and confirm your email address in order to receive an email confirmation of your request.
- 7). Enter the credit card holder's name exactly as it appears on the card.
- 8). Select the card type using the drop-down menu.
- 9). Enter the card number and security code in the corresponding fields.

Use the drop-down menu to select the month and year the card expires. - 11
Select the card's billing address from the list of available addresses, or select "Enter a different address" to enter another billing address. - 12
Select the box verifying the accuracy of the information you have entered. - 13
Select "Submit" to complete the request.
