CT revealed pansinusitis with extension into the orbit and a subperiosteal abscess along the superior lateral orbit (Figure 3).

The patient was admitted to the hospital for broad-spectrum antibiotics and sinus lavage. He was observed closely for improvement or progression of disease. After 24 hours, he had a slight decrease in vision, and the decision was made to drain the subperiosteal abscess. An incision in the upper eyelid crease was used to access the superior lateral orbit. The abscess was drained and cultured. Cultures revealed a bacterial infection of mixed flora. After 48 hours, the patient was significantly improved and was discharged home.
