How to Get My Pharmacy Records

    • 1). Request a copy of your pharmacy records. Some pharmacies will accept the request by phone or in person; others require that you complete a records request form. You must submit the patient's full name, date of birth and address on the form or to the representative.

    • 2). Go to the pharmacy and pick up the record. Some pharmacists will send the record to your home upon request.

    • 3). Submit proof of identity or the right to the record. Right to the record includes guardianship papers, power of attorney or a birth certificate. If sending the request, make a copy of this proof and submit it with the form, by mail. If requesting the record in person, show proof.
