- 1). Obtain Form 1040X from the IRS website.
- 2). Fill in the first four lines with your current information. On line A, check your filing status. On line B, check the year of the return you're changing. On line C, explain the reason for the changes.
- 3). Fill out the section on income and deductions with your adjusted gross income and itemized deductions. If you are changing your itemized deductions, fill out Schedule A. If you are changing your exemptions, fill out the second page of the 1040X.
- 4). Complete the tax liability section with information from the IRS tax table. That table is available in the 1040 instructions on irs.gov.
- 5). Complete the sections on payments, then calculate the refund you are due or the amount you owe. If you owe taxes, fill out a 1040-V payment voucher.
- 6). Attach all pertinent information, including any corrected W-2 forms and a check if you owe taxes, to the 1040X and send to the nearest IRS Center. Copy all the information you will be sending and keep it in a safe place.
