Those Overweight Kids: What Is a PCP to Do?

How Would You Assess This Child?


William is a 5-year-old black boy who has been a patient in your primary care practice since birth. He presents today for a health-maintenance visit. He was seen regularly for well-child evaluations during the first 24 months of life. However, since then he has only presented for acute-care visits and follow-up evaluations for his frequent episodes of acute otitis media. He has had tympanostomy tubes placed twice because of persistent serous otitis. His history is also significant for mild intermittent asthma and atopic dermatitis, both of which are currently well controlled. He has been healthy otherwise, his growth has been on track, and he has received all of his routine immunizations.

Today his height is 107.5 cm (50th percentile) and his weight is 20.2 kg (75th percentile); body mass index (BMI) is 17.5 kg/m (87th percentile).
