How to Write an Addendum to a Policy

    • 1). Title the addendum. You can write something as simple as "Addendum to Policy," but because you may want to add additional addenda later, it's better to use a title that is more specific, such as "Addendum to Policy: Employee Breaks."

    • 2). Specify the policy to which the addendum is attached and the dates involved with the policy execution and termination. For example, you could write something like, "This addendum references Company X Employee Handbook, 2011 Edition, between John Doe Management and Employees X, Y and Z of the Accounting Department, in effect January 1, 2011, and terminating December 31, 2011."

    • 3). Quote the terms of the policy to be altered via the addendum under the heading "Previous Terms and Conditions" or something similar.

    • 4). Specify the alterations to the previous policy under the heading "Changes to Cited Terms and Conditions" or something similar.

    • 5). Indicate ramifications and purpose of the policy for clarity -- that is, explain in layman's terms how the policy will impact all parties involved and what the objective is for the company. You may omit this step if desired, but it often helps all parties understand the company's bottom line.

    • 6). Indicate the date on which the addendum terms are to become effective, as well as the date of termination. Typically, the addendum termination date is the same as the termination date for the original policy, but it may not be, depending on the purpose of the addendum. State that all other terms of the policy remain unchanged and in effect.

    • 7). Provide a clearly labeled space for all parties to sign and date the addendum. Ideally, there should be spots for at least two witnesses to sign and date, as well.
