- 1). Complete Form SS-5, the application for a Social Security card from the Social Security Administration. Download the form from the SSA website or pick one up in person at the Minneapolis SSA office location.
- 2). Include your name, address, phone number, date of birth, sex, ethnicity and the names of your parents. Sign your application.
- 3). Gather your supporting documents. You must provide two forms of identification that prove your age, citizenship and identity. The SSA accepts birth certificates, naturalization certificates, certificates of birth abroad, military and government identification cards, driver's licenses and photo identification cards and passports.
- 4). Take your application and supporting documents to your Minneapolis SSA office (no website; 330 2nd Ave. South #650, Minneapolis; 800-772-1213). You only have to submit the documents to an employee; there is no fee for obtaining a Social Security number. Your Social Security card will arrive in the mail one to two weeks later.
