- 1). Request an Application to Replace Permanent Resident Card, form I-90, from your local U.S. Citizenship and Immigration Services office or its website.
- 2). Read and complete each section of the application form. Write clearly in black ink or type in your answers.
- 3). Go to Part 1, No. 1 "Your Current Legal Name." Check the appropriate box for your name change under Part 1, No. 2 "Has your name legally changed since issuance of your Permanent Resident Card." Write your full name as it appears on your current card before your name changed under Part 1, No. 3 "Your name exactly as reflected on your Permanent Resident Card."
- 4). Go to Part 2, No. 2 "Reason for application" section. Check "My name or biographic information has been legally changed since issuance of my existing card." Submit a copy of the original court order with your current name, marriage certificate, divorce decree, birth certificate, passport or other legal document that reflects your correct name.
- 5). Include a check or money order for the application fee and biometric fee, if your card is expired or will expire within six months. Sign the application form and mail it to:
