How to Get an Infant a Social Security Card in Howard County, Maryland

    • 1). Go to any of the Howard County Social Security Administration Offices to obtain a copy of form SS-5, the application for a Social Security Number and card. You can also print this form online (see Resources). Use the Social Security Administration's local office search page to find the Howard County SSA office nearest to your home (see Resources).

    • 2). Enter the name of your infant, his date of birth, location of birth, citizenship, ethnicity and parents' names. Including the parents' Social Security Numbers is optional.

    • 3). Sign form SS-5 and check the box that applies to your relationship with the child. Take form SS-5 to any Howard County Social Security Administration office, along with a certified copy of your infant's birth certificate to prove her age, identity and citizenship.

    • 4). Submit your application to the Social Security office. Once your child's application is submitted he will receive his Social Security Card through the mail within two weeks.
